Specialty care
that doesn’t
stop when the
visit ends

Codex is a virtual specialty clinic for chronic cardio-kidney-metabolic conditions. Our specialist-led team treats related conditions together, supports patients in their daily lives, and keeps referring physicians informed.

Refer the patient,
not just the diagnosis

Hypertension, diabetes, obesity, kidney disease, and heart disease all drive one another. Referring each condition to a different specialist leaves your patient on separate waitlists, with separate plans, and no one but you looking at the whole picture.

Codex treats all these connected conditions together, all with one team and one care plan.

Move your patient from the waitlist into care

Wait times for traditional specialists typically run 3–6 months. Dr. Calvin Wu leads a multidisciplinary team of physicians, registered dietitians, nurses, health coaches, and behavioral health specialists who can begin caring for your patient within days.

  • “First off, I want to start just by saying thank you, and to my doctor for getting me into the program with you all. I think it’s just incredible. It is really great to be able to monitor my blood pressure and work with a clinical team that syncs with my primary doctor, all from the comfort of my home. If this is a little taste of the future of medicine, I am all in.”

    Codex Patient

  • “Everything’s good. I had blood work done, and it’s in great shape. I’ve seen my cardiologist, I saw my regular family doctor, and they’re both very pleased with my progress. So everybody’s happy. My doctors are thrilled that I decided to join Codex.”

Frequently asked questions

Does my patient stay my patient?

Yes. A Codex referral is a specialty consult. You remain the primary doctor. We send you a note after every visit and keep you current on their progress.

What does this cost my practice?

Nothing. Codex works with the patient’s insurance. You do not take on billing, staffing, or financial risk.

How do I refer?

Complete the form on this page or send a referral through your usual process. If you want EMR integration or regular practice reviews, use Discuss a partnership and we will set that up.

How soon can you see my patient?

Most patients can book a virtual visit within days.

What conditions do you treat?

Codex manages the cardio-kidney-metabolic cluster: hypertension, prediabetes, type 1 and type 2 diabetes, obesity, dyslipidemia, chronic kidney disease, cardiovascular disease, Class I/II heart failure, metabolic dysfunction-associated steatotic liver disease (MASLD), and obstructive sleep apnea. Codex is not a substitute for procedural cardiology, nephrology, or hospital-level care.

Do patients need a referral to be seen?

No. You can refer them, or they can contact Codex directly. With the patient’s authorization, we keep their primary care doctor informed through visit notes and progress reports.
